• How has your mood been lately?

Sometimes changes in our mood can sneak up on us—feeling down more often, losing interest in things we usually enjoy, having less energy, or just not feeling quite like ourselves. Stress, life changes, and everyday demands can all play a role.

This quick check-in is here to help you pause and take a closer look at how you’ve been feeling.

We’ve partnered with providers and organizations to offer a brief, confidential screening to support awareness around depression and overall well-being.

This isn’t about putting a label on how you feel—it’s about understanding.

Taking a moment to check in with yourself can be a powerful first step.

     How It Works

    • 10 simple questions about how you've been feeling recently
    • No right or wrong answers—just an honest check-in
    • Takes about 2–3 minutes to complete
    • Your responses are kept private and are not linked to your name
    • Results are just for you—to help you better understand your current patterns

    What You Need to Know

    • This screening is part of a mental health initiative focused on awareness and education. It is not a diagnosis and does not replace medical care.
    • Your responses and total score are recorded in a de-identified way, meaning they are not connected to your name or personal information.
    • At the end of the screening, you will have the option to download, print, or save your results. If you choose, you may share them with a healthcare provider or behavioral health professional.
    • Your results are based only on your responses and may not reflect your full health history or current needs.
    • If your results suggest a higher level of risk, or if you have concerns about your mental health, we encourage you to talk with a healthcare provider or a qualified professional.

  • For each question, select the answer that best reflects how often you’ve experienced each concern over the past two weeks. After you complete all 9 items, your score will be calculated for you.

Please remember, this is a screening - not a medical diagnosis. Your score is only one part of the picture. If you’re concerned about how you’ve been feeling, talk with your health care provider regardless of your score.

  • Over the last 2 weeks, how often have you been bothered by any of the following problems?

  • Little interest or pleasure in doing things
  • Feeling down, depressed, or hopeless
  • Trouble falling or staying asleep, or sleeping too much
  • Feeling tired or having little energy
  • Poor appetite or overeating
  • Feeling bad about yourself — or that you are a failure or have let yourself or your family down
  • Trouble concentrating on things, such as reading the newspaper or watching television
  • Moving or speaking so slowly that other people could have noticed? Or the opposite — being so fidgety or restless that you have been moving around a lot more than usual
  • Thoughts that you would be better off dead or of hurting yourself in some way
  • If you checked off any problems, how difficult have these problems made it for you to do your work, take care of things at home, or get along with other people?
